What the Trials Really Say About Stopping GLP-1s

And why your weight isn’t the only thing you should be watching

If you’ve read the headlines – or found yourself deep in a late-night Google spiral – you’ve seen it:

 

“Everyone regains weight after stopping GLP-1s.

”The hunger comes back immediately.”

“Maintenance is impossible.”

 

Let’s pause that noise.

 

Yes, the trials showed weight regain

⮞  But no, that doesn’t mean it’s inevitable for you.

What the research actually says

Major trials like STEP-1 (extension), STEP-4 (semaglutide), and SURMOUNT-3 & -4 (tirzepatide) did show weight regain when participants stopped their GLP-1 medication. In some cases, it was rapid and significant.

 

But here’s what those trials didn’t include:

  • No physiological assessment of whether participants' bodies were ready to stop
  • No structured tapering or transition plan
  • No mindset support through the withdrawal phase

 

In STEP-1 (extension) and SURMOUNT-4, participants were simply taken off the drug with no further support at all. Others were given basic calorie-control strategies.  You know - the ones which have been around for years and not helped you in the past.

That didn’t work for them, either.

 

So let’s ask the better question: Is it any wonder they struggled?

 

Because what we know now is this: Weight maintenance after a GLP-1 isn’t just about calories in vs. out. 

It’s about biology, behaviour, and belief.

It’s about your body’s readiness.
